Benefits, risks & the future of patient care

In the chaos of an emergency scene where decisions are made in seconds and outcomes hang in the balance, there is one certainty for every EMS provider: an ePCR must be completed. The electronic patient care report — though essential for continuity of care, quality assurance compliance and billing — is also one of the most time-consuming and frustrating parts of the job.

Enter artificial intelligence (AI)

AI is rapidly emerging as a transformative force in EMS documentation.Tools powered by natural language processing (NLP), predictive text generation and voice recognition promise to dramatically reduce documentation time, streamline accuracy and unburden overworked clinicians.In today’s world of rising call volumes, reimbursement decline and staffing shortages, technology isn’t just helpful, its essential. However, as the ePCR becomes smarter, faster and more automated, the EMS industry must ask a critical question: are we enhancing care, or outsourcing responsibility?

The peril of progress

Behind the convenience of AI lies significant complexity. AI may simplify the “how” of documentation, but it can also undermine the “why.”Writing a narrative is more than recordkeeping; it is clinical storytelling.

It is a vital component of patient care, transforming a series of observations and interventions into a coherent narrative that captures the full context of a patient’s experience. It allows providers to articulate their clinical reasoning, justify treatment decisions and communicate critical information to other members of the healthcare team.

When ePCRs become overly automated, there is a risk that this narrative depth is lost; reduced to checkboxes, template phrases or AI summaries that lack the nuance of true clinical judgement.

This can lead to a diminished understanding of the patient’s condition, a weaker continuity of care, and an erosion of the providers’ critical thinking skills. Over time, reliance on automation may shift the focus from patient-centered storytelling, to system-driven data entry, ultimately impacting the quality, empathy and accountability at the heart of effective clinical care.

Automation bias is our tendency to over-trust machines even when other information suggests the recommendations are incorrect, which can create false confidence and dangerous blind spots. Think of how many times the GPS turned you in a direction you knew was wrong.

When the machine gets it mostly right, it becomes seductively tempting to trust it completely. However, what happens when AI misinterprets a dictated phrase, suggests the wrong medication or omits a crucial nuance? Providers may begin to overlook errors, rubber-stamp suggestions or even defend faulty documentation because “the system did it.”

Then there is the issue of cognitive offloading — a phenomenon where individuals delegate mentals tasks, such as analysis, decision-making and problem-solving to technology instead of engaging in them directly. Preliminary research has found that excessive reliance on AI tools can weaken internal cognitive abilities, such as memory retention, critical thinking and independent decision-making over time. While research is still preliminary, our addiction to technology has become obvious, making the threat more than mere hyperbole.

Shaping the future of EMS

Technology should enhance human judgement, not replace it — and in EMS documentation, the role of AI should be to augment, not automate. This means maintaining strong human oversight, where AI-generated reports are always reviewed, edited and signed by the practitioner.

However, oversight is only meaningful if the reviewer first understands what quality clinical documentation looks like. EMS providers must continue to train in manual documentation, not only to preserve clinical reasoning and narrative skills, but also to ensure they are equipped to identify errors, omissions, commissions and bias in AI-generated narratives.

If practitioners are expected to police the output of AI, they must first master the craft themselves. Agencies should also conduct regular audits to detect bias in AI outputs and develop clear policies that define ethical use, accountability and data governance.

AI is not a passing trend and is quickly becoming woven into every layer of EMS, from dispatch to documentation. However, AI is neither angel nor demon; it is a mirror, reflecting the values and intentions of those who build and wield it.

In EMS, our mission remains the same: serve with compassion, act with integrity and do no harm. As AI beings helping us tell the story of patient care, we must ensure that the story still carries a human voice.

How EMS leadership responds today will define how AI shapes our organizations and impacts our patients tomorrow. That is why it is essential for leaders to take an active role in developing the policies, guardrails and implementation strategies that govern AI use. If we do not lead its integration with intention and clarity, we risk letting it lead us, and we may not like where that is.
